This assessment should be completed with a client from your field placement (if you are not in field, you may use a volunteer, past client, movie, book character). In this assignment you will provide a biopsychosocial assessment of your client using the assessment guide (attached). Use the outline format on the assignment guide; you should address each letter and number on the outline even if the area is not applicable to your client. Include headers/subheaders for each section. An eco-map must be attached to your paper that reflects the systems and barriers the client is navigating. This is completed in section 5 and on a separate page.
The goals you create must be focused on gain instead of deficits. When writing goals, consider the content you have learned throughout the course. You are encouraged to emphasize strengths and person in the environment. Remember that goals are often read by many treatment team members, so include language that can be read and understood by a non-social work person. Stay in one tense while writing goals and make sure they are in complete sentences.
You must include at least 2 peer-reviewed articles when completing section 3. The paper should be double-spaced and a maximum of 12 pages. 5 points will be deducted if paper goes over the 12 page limit (title page and reference list are not included in page count). The assessment is worth 100 points and 30% of your final grade in the course. Your paper will be evaluated according to the below rubric. Use APA Guidelines (e.g., one-inch margins, a 12-point Times New Roman font, APA-style citations, etc.). The paper should be double-spaced and a maximum of 12 pages. E. Social Factors and Cultural factors

4. Significant cultural factors, and how these affect the client (consider race, religion, ethnicity, age, gender, sexual orientation, disability, community norms and values)
5. Cultural norms and their integration or coexistence with wider societal norms; attitudes about receiving help as influenced by cultural norms
F. Spirituality
1. Religion/Spiritual practice (consider impact on activities, source of conflict or identification with family of origin, whether membership or lack of membership is source of support or conflict)
2. Beliefs/value/morals/value base/world view (consider affective and cognitive aspects of beliefs)
G. Assessment Summary
1. Provide a summary of the identifying information and problem(s).
2. Discuss strengths
a.) Include client’s capacities (consider ego strengths, skills, models of coping, motivation, hope, resiliency, ability to access external resources, understanding of the current problem, current problem-solving resources)
3. Discuss resources available for problem resolution change and growth
a.) Include availability and commitment of informal and formal support systems and service systems
Section 2: Identify the problem(s) that your client is experiencing. This must include describing the development of the problem, including intensity, frequency, duration, consequences, and reinforcers.
For example, if you are seeing a client that is experiencing prolonged grief, behavioral definitions of the problem may include the following: avoiding places and people that remind them of their loss, poor concentration, loss of support, difficulty accepting loss, and preoccupied thoughts.

Create one long-term goal, two short-term goals, and two objectives per short-term goal.

Identify the target of intervention (who is being helped)
Identify modalities for intervention and the rationale for these modalities (individual, family, group, and/or milieu)
Section 5: Create an eco-map and attach it to a separate piece of paper.
