Current Presentation: Aaron is a 45-years-old, African American man who lives in rural Missouri. He comes into his first appointment reporting that he has painful peripheral neuropathy in both of his lower legs and feet. Aaron tolerated the initial feelings of numbness and tingling of the neuropathy in his legs, but now the pain has become unbearable. He also reports that he feels as though his thinking is “foggy” sometimes, and as a result, he has trouble remembering details from conversations. The family has noticed that Aaron is more irritable than usual and somedays misses work or church because he struggles to get going in the morning. When his daughter asks him if he will go see a doctor or talk to a social worker about his low mood, he tells her not to worry about him, puts on a smile and pushes through the rest of the day. When his new wife shared that she is worried about Aaron’s health, Aaron says that he’s strong, and burns off plenty of calories working in and around his parent’s home on the weekends.
History: Aaron was diagnosed with diabetes 5 years ago. He worked for years as a full-time truck driver, and now works part-time at the local public library. He is a devoted family man and leader in the community. He cares for his parents – helping them with chores, house repairs, and paying for their rising property taxes. He serves as deacon and bible study leader at his church, and sends any extra income he makes to his daughter who is the first in the family to attend college. Aaron has either been uninsured or underinsured for the last 5 years. His pattern was to only go to urgent care when he was feeling really ill, get prescriptions for his diabetes, and then, ration the medications as long as he could. His part-time library job is where he met “the love of his life.” Once they married, Aaron had access to his partner’s employee-based insurance, which is why he is now able to come to see you.
