It is 11am and you are on placement in an Orthopaedic Trauma Ward.
Dimitri injured his right ankle while playing football last night. He was diagnosed with an unstable fracture which required surgical repair (open reduction & internal fixation- ORIF), under general anaesthetic. He has just returned to the ward from theatre and has a below knee plaster cast in situ. Dimitri works as a chef, has well controlled asthma and reports that he is normally fit and well. He received 1 gram of intravenous paracetamol while in theatre (2 hours ago). On return to the ward, he is lying flat in the bed. He appears anxious, has vomited once and called the nurse to state that his pain has increased.

Margaret lives with her husband who states that she is awaiting investigation at the memory clinic and is a little confused but there has been no recent change. Margaret takes regular furosemide and has just finished a course of antibiotics for a urinary tract infection but is still experiencing urinary frequency and some discomfort.

Vijay Kumari is a teacher and is experiencing an exacerbation of his Inflammatory Bowel Disease (IBD. He is currently on a course of prednisolone. Since having his routine review appointment 3 months ago, he has noticed some unintentional weight loss and his partner has commented on how his clothing has started to look “too big”. He has a decreased appetite, abdominal pain and an increased frequency of loose stools for a few weeks but reports nausea and significantly worsening diarrhoea over the last few days.
