You will be conducting a full behavior evaluation plan for a student and developing an intervention plan with implementation guidelines. Choose either Amy, David, OR a Focus Student (FS) you are currently working with who would benefit from this assignment. Begin by reading through the Resources including Writing a Positive Behavior Support Plan. If you are using Amy or David in the case studies, extrapolate and make a reasonable guess on what behaviors you might see, why they are exhibiting the behaviors, and how you might best address them. Adapt Amy or David’s personas and backstories to your grade level and MMSN or ESN disability groups to maximize personal relevance and learning from the assignment.
Complete the following:
Start with the 1 Environmental Observation Form 082923 for your Focus Student (FS), Amy, or David based on the videos and extrapolating as needed. This excellent form can help you learn more about the student, especially how the environment might be impacting the student’s behavior. SUBMIT the completed form as part of your field notes 1.
Data is important. Use the 2 Behavioral Record & Type of Behavior Plan 040523 for observing and writing data. Determine if you’re describing single or multiple behaviors with single or multiple functions. Based on your choices of behaviors and functions, complete either the first or second box of the Behavioral Record, and then select a Type of Behavior Plan depending on the function or functions of the behavior or behaviors. Take a look at it to see the variety of approaches from simple and complex behavioral plans. Use one line per behavior; note if they have different antecedents and consequences. Make at least three different dates and timed entries to demonstrate that you understand the ABC data collection process. Follow above instructions for Amy or Joe’s case studies. SUBMIT the form as field notes 2.
Now that you have observed and collected data, it is time to move Amy, David, or your FS to Tier II. Follow above instructions and extrapolate for case studies. Review this descriiption of the Three Pathways Summary Chart. Complete the 3a Three Pathways Chart with details and SUBMIT as field notes. NOTE that the box for Maintaining Consequences is about why the student uses and persists in the behavior, i.e., the payoff for the behavior. What does the student get out of the behavior?
Describe what types of 4 Curricular Adaptations bsp NineTypes_guide you would implement to support Amy or David and SUBMIT your plans on the 4a Individual Curriculum Adaptations form.
Write at least 2 goals, ONE to increase positive/decrease negative and ONE replacement behavior for Amy, David, or your FS using the 5 BSP goalwritingguide rv and SUBMIT as field notes less the explanations and examples. Follow above instructions for case studies and delete the explanations and sample goals. SUBMIT the form as field notes 5.
Complete the 6 Action and Evaluation Plan 2303 extrapolating as needed for Amy, David, or your FS. The first column for Tasks should describe your strategies for Prevention, Teaching, Extinction, etc. The next column lists who would be responsible for implementing the strategy, followed by two time markers. The last column after the Review Date, titled Evaluation Decision, can be left blank as that’s for follow up after the plan is in place. Evaluate the plan on the next page and make sure to complete the communication plan. Follow above instructions for case studies. Write a 4-8 page Integrative Paper that:
Identifies the specific additive value of each document. What did you learn about the student from each document?
Describes how your experience with developing a Positive Behavior Support Plan will support your current and/or future students.
Make sure you upload 6 forms numbered 1-6 as attachments to the submission; it helps to list the name of the documents to check that you’ve uploaded the completed forms. Double check that you are not submitting blank forms!
